REGISTRATION INSTRUCTIONS FOR ACCEPTED TEAMS

ON-LINE REGISTRATION ONLY:

Check – In Online is due by August 17th

There will be no exceptions or extensions allowed!

INSTRUCTIONS ON HOW TO REGISTER ON-LINE:

1. Fill  in and print the PDF document labeled Team Information Sheet.

2. Documentation:

a. US Club Teams need to provide:

i. 1 copy of the US Club Roster (see “Frequently Asked Questions” below about adding guest players)

ii. A copy of each US Club Players Passes (ONLY front must be copied)

iii. A copy of each guest player's US Club Player Passes (ONLY front must be copied)

iv. If passes are not available a photo id and a birth certificate will be accepted.

b. US Youth Soccer Teams need to provide:

i. 1 copy of the State Approved Roster (see “Frequently Asked Questions” below about adding guest players)

ii. A copy of each US Youth Soccer Player Passes (ONLY front must be copied)

iii. A copy of each guest player's US Youth Soccer Player Passes (ONLY front must be copied)

iv. If passes are not available a photo id and a birth certificate will be accepted.

v. Teams outside of Region 1 need to provide a signed permission to travel form. Check with your State association for the appropriate form.

(Region 1 teams include: ME, NH, VT, MA,NJ, RI, PAE, PAW, NYE, NYW, CT, MD, DE, VA, WV)

c. Foreign Teams: For a team coming from a CONCACAF nation: 
•    Players must present passports at registration or, if from a nation that the United States does not require a passport, proof of entry into the United States that is required by the United States. 
•    Teams are required to have and present player picture identification cards. 
•    Tournament rules require that the team have a completed form from its Provincial or National Association approving the team’s participation in the tournament.

v. Print, read and sign the Tournament Policy/Acknowledgement Form.

To complete the electronic registration please follow these instructions.

 

·       Login to your GOTSoccer team page - https://www.gotsport.com/asp/users/login_menu.asp

·       Click on the "44th Annual East Meadow Soccer Tournament" event located under "Event Registration History"

·       Click on the "Documents" tab on the far right

·       Load Documents

 

The following documents must be uploaded into your team's GOTSoccer account in PDF format no later than Friday, August 19th.

NO EXCEPTIONS!

 

  • Team Roster
  • Player Passes (if not available please upload player ids and birth certificates)
  • Medical Waivers ( All waivers should be scanned in one PDF)
  • Team Information Sheet
  • Tournament Policy/Acknowledgment Form
  • Permission to Travel​ (if applicable)

Once all of your documents are uploaded, click on "Notify" next to Notify Registrar

 

REGISTRATION FAQ

If a player is not attending the event do I leave their name on the roster?

No, if a player is not attending the event; you need to cross their name off of the roster.

Guest Players:

What is the maximum number of guest players a team can bring to the event?

A max of three (3) or four (4) guest players will be allowed. Please refer to Tournament Rules for your specific age group.

If a team has no guest players, USYSA teams may roster up to the maximum permitted for their age and US Club Soccer teams may roster up to 25 players. A maximum of three (3) guest players will be allowed on a team but only to bring the number of players on the approved roster for the tournament to a maximum of 18 (for 11v11) or 15 (for small-sided divisions). That is, no guest players are permitted on any team with a tournament roster of more than 18 or 15 respectively. E.g. if an 11v11 team has three guest players, it may have no more than 15 regularly rostered players.

How do I add a guest player?

Step 1: Obtain the guest players current player card and medical release form

Step 2: Copy your original roster. Please handwrite the guest player’s full name, unique

jersey number, player identification number and birthday below the bottom of the existing

rostered players.

Note: US Club Soccer teams may only take guest players registered under US Club Soccer, and USYSA teams may only take guest players registered with properly stamped USYSA player pass cards. No roster may be comprised of players with different passes from different organizations (no “mixed rosters”).

How do I add a guest player after I emailed my registration information?

Due to the size of the tournament, teams will not be allowed to resubmit their roster online after the deadline. Instead teams will be allowed to resubmit their roster during the on-site check-in dates and times. If your team resubmits their roster they must follow the directions for on-site registration.

Medical Release Forms

Do I include the medical release forms in the early registration document?

All teams must have a medical release form for each player (including guest players).
